During the registration process you will be asked to provide information about yourself which will include certain demographic and financial information as well as general consent and insurance authorizations.

On the day of your registration, please bring the following items as they pertain to your specific situation:

• Insurance identification card(s)
• A photo identification card of the patient
• Social Security Card
• Living Will, if you have one
• Emergency notification of someone outside your home
• Minor patients must be accompanied by parent or legal guardian

We ask that you leave your valuables at home or send them home by family or friends. The hospital is not responsible for valuables kept in your room.
